Spinal Energetics Informed Consent & Waiver

Spinal Energetics is a body-based, nervous-system-focused practice that may include movement, breathwork, guided imagery, ritual, and energetic or intuitive work. It is offered for educational and personal growth purposes and is not a substitute for medical care, psychotherapy, or psychiatric treatment. Nothing in this membership should be treated as diagnosis or treatment of any condition.

Because this work can bring up strong physical, emotional, and nervous-system responses, certain health histories and conditions warrant a conversation before joining so we can make sure this is a safe and appropriate fit for you. 

By checking the agreement box below and signing your name in the form, you confirm that:

1. You have honestly disclosed all relevant health history.
2. You understand this work is not medical or mental health treatment, and you will continue any care from licensed providers as needed.
3. You participate voluntarily and assume full responsibility for your own physical, emotional, and psychological wellbeing during and after sessions.
4. You understand that if you disclosed any condition above, your membership will not be activated until you and Allison have discussed it and she has confirmed it's appropriate to proceed.
5. You release Allison and Heal With Allison from liability for any claims, injuries, or damages arising from your voluntary participation, except in cases of gross negligence.

If anything changes in your health history after joining, you agree to inform Allison before continuing.
